Child Health Plan Plus (CHP+ Health Insurance)


Child Health Plan Plus (CHP+) is low-cost health coverage for uninsured Colorado children age 18 and younger whose families earn too much to qualify for Medicaid but cannot afford private insurance. The CHP+ Prenatal Care Program is a Colorado program for uninsured pregnant women 19 and older who meet financial and other qualifications.

How Do I Enroll My Child?

Larimer Health Connect provides free, in-person assistance to help determine your eligibility for Medicaid and CHP+ and help you apply for those programs if you qualify. Call 970-472-0444 to make an appointment. Se habla Espanol.

Larimer Health Connect
Applications may be picked up at the Larimer Health Connect office at 144 N. Mason St., Unit 7, in Fort Collins. Free, in-person assistance in filling out the application is also available.
